How to Schedule a Semen Analysis or Sperm Cryopreservation Appointment
If you are a current Boston IVF patient and wish to book a semen analysis or sperm cryopreservation appointment, please contact your Boston IVF doctor’s administrative assistant (888) 300-2483 and press Option 1.
If you are NOT a Boston IVF patient and wish to book a semen analysis, you must have your doctor (primary care, urologist, etc) fax a lab order to our office at (781) 370-0099.
Please make sure the lab order contains your name, date of birth, and phone number along with the requested testing. Once this lab order is received, you will receive a call from Boston IVF to book your appointment. Please allow several days for processing. If you need to contact our office, please call (888) 300-2483 and press Option 2.
If you are NOT a Boston IVF patient and wish to book a sperm cryopreservation appointment, please be advised that we offer these appointments on a case by case basis to patients recently diagnosed with cancer or to patients about to undergo gender affirming hormone treatment. Your doctor (oncologist, primary care, urologist, etc) must first fax a lab order for sperm cryopreservation to our office at 781-370-0099. Please be sure the lab order contains your name, date of birth, and phone number. Once this lab order is faxed, please contact Boston IVF at (888) 300-2483 and press Option 2.
